Residence Life Forms
Roommate Questionnaire
After you have submitted your Housing Reservation Contract and paid your damage/surety deposit, the next step is to complete the Roommate Questionnaire. You will state your preferences for the type of person you would be most compatible living with. What kind of music do you like or what hobbies/activities interest you? Do you like your spaces neat? Are you a morning person or a night owl? If you have a specific person who you would like to room with, be sure to put that person’s name down and have them do the same for you. Roommate requests will only be honored if both persons mutually agree.

Immunizations
ÅÝܽÊÓƵapp Community College currently does not mandate immunizations to live on campus. However, being up-to-date on all immunizations is certainly encouraged.
Meningococcal Disease: In 2003, the Nebraska State Legislature passed a law mandating all students entering college receive information regarding meningococcal disease, a rare infection with potentially devastating results. Meningococcal disease is a serious illness caused by a type of bacteria called Neisseria meningitidis. It can lead to meningitis (infection of the lining of the brain and spinal cord) and infections of the blood. Meningococcal disease often occurs without warning — even among people who are otherwise healthy. This infection occurs sporadically in clusters and is spread by close contact with infected individuals. Students living in residence halls are in a high-risk group.
There are two types of meningococcal vaccines licensed in the United States:
- Meningococcal conjugate vaccines (MenACWY)
- Serogroup B meningococcal vaccines (MenB)
These vaccines help prevent the most common causes of meningococcal disease in the United States.
All students will be provided information on meningococcal disease prior to check in and will be required to acknowledge receipt of this information. You may access this information in advance of checking in by completing the acknowledgement form (PDF) and either mailing it to the appropriate campus location or bringing the signed form with you when you check-in.
